A copy of the updated form is attached and is also available on the State of New Jersey Web Site DisclosureofProhibitedActivitesinRussiaBelarus.pdf (nj.gov), as well as at the City of Bayonne Law Department, 630 Avenue C, Room 15, Bayonne NJ 07002 during normal business hours.
