Care Management

Care Management is the development of a coherent care plan for arranging and coordinating the delivery of multiple services to vulnerable and/or functionally impaired older individuals. This is a community-based program that helps older residents remain home and delay or prevent institutionalization. Services can be paid for from a variety of public or private sources and includes a comprehensive assessment, care plan development, care plan implementation, monitoring and reassessment. Eligibility is based on financial needs as well as ability and desire to remain in the community with formal and informal supports. The plan is developed in cooperation with the client and/or the client’s guardian or designated representative. For more information, please call (201) 858-6119.
